Mattel Factory Inspection Documents List In order to ensure smooth review and good preparation before the review, the following documents should be collected before the first day of the review: 1. Employee records Labor contracts/employment terms and conditions List of employees Employee files (ID card copy, birth certificate copy, medical examination copy) 2. Employee Benefits 3. Factory Policies and Procedures 4. Internal Operations and Procedures 5. Business license 6. Maintenance and health license 7. Wage and working hours policy 8. Policy on Termination of Employment at Any Time 9. Anti-discrimination policy 10. Employee/Management Grievance Policy 11. Freedom of Association Policy 12. Other government licenses, operating permits 13. Wage records for the last six months. Wage records for off-season and peak seasons. Work cards or other work time records. Wage records. Wage receipts. Overtime calculation method documents. Labor department approval documents, such as extended overtime hours. 14. General environment, health and safety factory layout or workshop floor plan, communication records of regulatory authorities EHS training records (sign-in book, content outline, training materials) EHS manual and/or written EHS plan inspection records (fire extinguishers, eyewash/irrigators, etc.) 15. Environmentally friendly drinking water analysis report Hazardous waste record list or transportation document Waste property data sheet/test report/waste analysis 16. Health and Safety Material Safety Data Sheet (MSDS) Industrial hygiene testing records (chemical exposure and noise) Job/position hazard assessment or measurement Personal protective equipment (PPE) checklist Fixed and portable fire extinguishing equipment testing, inspection and maintenance records |
