HM Factory Inspection (Hallmark Factory Inspection) Document List

1. Floor plan of the factory;
2. A copy of the employee’s ID card;
3. Labor contract signed between the factory and the employee;
4. Receipts of purchased social insurance;
5. Letter of approval from the Social Security Bureau proving that the factory is exempted from purchasing social insurance items;
6. Business registration issued by the local government;
7.Tax registration certificate issued by the local government;
8. The local labor bureau approves the worker's overtime permit;
9. All employees should submit an application form with the employee's name, date of birth and place of origin;
10.a) Monthly salary list and attendance records,
b) Salary and attendance records for the past 12 months;
11. Fire inspection certificate issued by the Fire Department;
12. Factory rules and regulations on welfare, such as annual leave, maternity leave
13. Location map of fire escape routes;
14. Fire prevention and emergency response plan, fire drill records;
15. Professional qualification certificate or license of the on-site medical staff (if the factory has medical staff)
16. Employee work-related accident records;
17. Copy of health permit/medical certificate/electrician license;
18. Environmental impact assessment report;
19. Annual environmental monitoring report (if the factory has one)
20. Material Safety Data Sheets for all hazardous chemicals used in the factory (if the factory uses any)
